Each semester you will need to fill out certain forms and complete the appropriate coursework. Below is an outline of what needs to be done during the third work semester.
The pdf forms will initially open in a pdf viewer. You need to download the forms into the latest Adobe Reader in order to fill them out. All forms can be filled out electronically, with typed signatures.

You are advised to take no more than one other course, outside of the Co-op course, during your working semester. More than one requires permission from the Co-op director.

International Students: CPT Authorization Required
Follow the steps in the International Student Section regarding the extra required paperwork that needs to be completed before you can begin work. This paperwork needs to be completed each working semester.
It is very important to not delay this paperwork because it can take 2 weeks to get all of the letters and CPT forms signed and students cannot start working until CPT authorization is processed.

This form needs to be submitted to the CSE Office in 105 Lind Hall before paying tuition.
It needs to be filled out each spring and fall semester you are working. You do not need to fill it out for summer semester.

Complete the Work Verification - due 2 weeks after the start of the semester
Complete the 4044 Work Verification Form with your supervisor's typed signature and submit it to the Co-op office or the Moodle course site. The form is due two weeks after starting your third semester work term.
This form confirms that you are working full time during the listed dates. A grade will not be submitted until the verification form is submitted.
This form is different from the Work Verification Form submitted in 4043W.

This is an optional 3rd semester course. Computer Engineering students are not eligible for this course.
The Co-op course is an online course with one on-campus evening workshop. The coursework consists of:
- Technical PowerPoint presentation (15 to 20 minutes)
- Reflection paper about your Co-op experience

Work Experience Survey - due at the end of the semester
At the end of each working assignment, a Work Experience Survey should be completed by the student and returned to the Engineering Co-op Program Office. The survey is intended to provide an opportunity for students to rate his or her employer and work experience, thereby providing information that can improve the Co-op Program.

Performance Review - due at the end of the semester
At the end of each industrial assignment, a Performance Review should be completed by the student's supervisor and returned to the Engineering Co-op Program Office. Students are strongly urged to review their rating forms, preferably in person with their supervisor. If that isn't feasible they may set up a time to review it at the Co-op office. These forms are treated confidential and will not be released without written permission from the student.
If the supervisor does not initiate a review it is up to the student to request one. The information is used to screen for potential problems, but its primary function is to maintain good communication between the employer and the Co-op student. If a supervisor submits the form without talking to the student, the student has the option to set up a time with the Co-op office to look over the completed form.
A company-specific performance review form can be submitted in lieu of the Co-op's form.
